Sometimes the NPCs in NMZ can stack up on each other and get blocked, or they can take a while to reach you from their spawn, this happens more often if you stand in the corner of the room rather than the middle. This means that if you are doing guthans you will stop attacking and therefor healing after 20 minutes and die, once you die and leave NMZ you will no longer be under attack and you will log out. You are logged out after 5 minutes of inactivity, unless you are in combat. After I start a TeamViewer meeting its shuts off in a 1 minute.
